In the above mentioned case, you have to repeat the fast. Doing iftar mistakenly before time even by 2 minutes invalidates the fast; although there is no kaffarah in this state.

The answer to your question is as follows:
Yes, having sexual intercourse in the state of fast knowingly invalidates the fast, and it leads to both Qaza and Kaffarah. In case it is done unknowingly then fast does not break.
ومن جامع او جومع او اکل او شرف غذاءا او دواءا عمدا قضی وکفر ککفارۃ الظہار (کنزالدقائق ص 68)

Yes, Masturbating nullifies the fast. One has to make qaza of the fasts during which one committed masturbation, but there is no kaffara.

If people give something to Imam of Taraweeh on their own will, then it will be permissible. However, collection should not be made forcefully from the people for this purpose.

The answer to your question is as follows:
If a woman fasted in the month of shawwal to makes up her missed fast days of Ramazan (obligatory fasts) with the intention of Qaza, she has fulfilled her religious obligation and also obtained the reward of fasting 6 days of shawwal. But the woman must intend only to make up what she has missed from Ramadan.
Nevertheless, if she is in a good health, it better to fast the 6 days of Shawwal (Voluntary fast) separately.
أَقُولُ فِي فَتْحِ الْقَدِيرِ صَامَ فِي يَوْمِ عَرَفَةَ مِثْلَ قَضَاءٍ أَوْ نَذْرٍ أَوْ كَفَّارَةٍ وَنَوَى مَعَهُ الصَّوْمَ عَنْ يَوْمِ عَرَفَةَ أَفْتَى بَعْضُهُمْ بِالصِّحَّةِ وَالْحُصُولِ عَنْهُمَا (انْتَهَى) (غمزعیون البصائر 1/150)
